Understanding Glaucoma
Overview
Glaucoma is a complex group of eye conditions that cause damage to the optic nerve. Because the optic nerve is responsible for transmitting visual information from your eye to your brain, its health is vital for clear, comfortable vision. This damage is often linked to abnormally high pressure inside the eye, though glaucoma can also occur even when eye pressure is completely normal.
Often called the "silent thief of sight," glaucoma typically progresses without early warning signs. By the time visual changes are noticed, the condition is usually advanced. At Bespoke Eye Care, we emphasize early detection and proactive management to protect your long-term eye health.

Symptoms
The symptoms of glaucoma depend heavily on the specific type and stage of your condition.
Open-Angle Glaucoma
The most common form of the disease. In its early stages, there are virtually no symptoms. As it progresses, you may experience:
- Patchy blind spots in your side (peripheral) or central vision, frequently occurring in both eyes.
- Advanced stages may lead to tunnel vision.
Acute Angle-Closure Glaucoma
This is a medical emergency that occurs when the drainage angle in the eye suddenly closes. Symptoms come on rapidly and can include:
- Severe, sudden eye pain and headaches.
- Nausea and vomiting.
- Blurred vision and halos around lights.
- Noticeable eye redness.
Causes & Risk Factors
Your eye constantly produces a fluid called aqueous humor. In a healthy eye, this fluid drains out through a complex network of tissue at the angle where the iris and cornea meet. When this drainage system becomes blocked or inefficient, fluid builds up, causing internal eye pressure (intraocular pressure) to rise. Over time, this elevated pressure damages the sensitive nerve fibers of the optic nerve.
Who is at risk?
Because chronic forms of glaucoma can destroy vision before any signs or symptoms are apparent, it is crucial to understand your personal risk factors. You may be at a higher risk if you:
- Are over the age of 55.
- Have a family history of glaucoma.
- Have certain medical conditions, such as diabetes, high blood pressure, or migraines.
- Have corneas that are naturally thin in the center.
- Have extreme nearsightedness (myopia) or farsightedness.
- Have a history of eye trauma or have used corticosteroid medications for a prolonged period.
Diagnosis
At Bespoke Eye Care, diagnosing glaucoma requires a comprehensive, tailored evaluation of your overall eye health. Because subtle changes in the optic nerve can occur long before vision loss, we utilize modern diagnostic technology to monitor your eyes with precision.
- Tonometry: Measuring your internal eye pressure accurately.
- Optical Coherence Tomography (OCT): Advanced imaging that scans the optic nerve to detect microscopic, subtle damage to nerve fibers before they affect your vision.
- Visual Field Testing: Mapping your peripheral vision to identify any early functional loss.
- Pachymetry: Measuring your corneal thickness, which directly influences eye pressure readings.
- Gonioscopy: A specialized exam to thoroughly evaluate the drainage angle of your eye.
Treatment & Management
While the damage caused by glaucoma cannot be reversed, routine checkups and dedicated treatment can effectively slow or prevent further vision loss.
Every eye is unique, and our approach to Glaucoma Management is deeply personalized. Treatments may include:
- Prescription Eye Drops: The most common starting point. These medications either reduce the production of fluid or improve its drainage. (Note: We actively manage any dry eye side effects caused by glaucoma drops, ensuring your eyes remain comfortable.)
- Oral Medications: Prescribed in some cases when drops alone aren't sufficient to lower pressure.
- Laser and Surgical Therapy: When necessary, we utilize procedures such as Selective Laser Trabeculoplasty (SLT) or minimally invasive glaucoma surgeries (MIGS). We coordinate and co-manage surgical treatments with trusted specialists when appropriate.
